
MAPLE’S PLACE DOG HOTEL AGREEMENT & RELEASE FORM

Owner/Guardian I am the legal owner or guardian of my dog and fully authorized to enter this agreement.
Refusal of Service: Maple’s Place may refuse service to any dog at any time for health, safety, or behavior reasons.
​
Dog Requirements: Dogs with a history of aggression toward people or other dogs cannot be boarded. Please bring enough of your dog’s food for the entire duration of their stay. If your dog runs out, we will purchase more of the same brand when possible, and you will reimburse us. If the same brand is unavailable, we will select a high-quality substitute.
We do accept intact dogs. However, if your dog comes into heat while boarding, you will be responsible for the additional costs of baths, diapers, or other necessary accommodations.

Health & Medications: We are not veterinarians. We will administer medications as directed, but the owner accepts all risks. Please note that vaccines do not protect against all illnesses.
Fleas, Ticks & Parasites: All dogs must arrive clean and free of fleas, ticks, and internal parasites. If Maple’s Place discovers fleas, ticks, or worms during your dog’s stay, we will notify you immediately and take necessary steps to treat the issue. Your dog may need to be bathed, treated, and separated from other dogs until fully cleared to prevent spread. Any related treatment, cleaning, or separation fees will be added to your final invoice.

Veterinary Care: If your dog appears ill or injured, Maple’s Place may obtain veterinary care and transportation at your expense.

Play & Socialization: Dogs may spend time around or with other dogs during their stay. We have extensive experience with dog behavior and make every professional effort to group dogs thoughtfully based on temperament, size, age, and energy levels. However, dog behavior can change without warning, and fights can occur unexpectedly, even between dogs that have previously gotten along. Nips, bites, scratches, or scrapes may happen despite every effort to prevent them. By signing this agreement, you acknowledge and accept these natural risks of dog interaction
Aggression & Behavior: The safety of all dogs and staff is our top priority. If a dog displays aggression toward humans or other dogs, behaves in a way that makes handling unsafe, or becomes destructive to property beyond normal wear and tear, the owner will be promptly notified. In such cases, the owner may be required to find alternate accommodations within 24 hours of notification. If immediate removal is not possible and the dog must remain for the duration of their stay, they may be kept separate from others for safety, and an additional handling and care fee may apply to cover the extra time, supervision, and resources required.
Natural Disasters & Emergencies: In the event of severe weather, fire, natural disaster, or other emergency beyond our control, Maple’s Place will make every reasonable effort to ensure your dog’s safety. However, the owner understands that unforeseen circumstances such as power outages, tornadoes, or fires may impact operations, and Maple’s Place cannot be held liable for damages, delays, or injuries caused by such events.
Infectious Disease & Illness Outbreaks:

Although Maple’s Place maintains high sanitation standards and keeps capacity limited to reduce exposure, infectious diseases such as kennel cough, canine flu, or other airborne illnesses can occur despite vaccination and cleaning protocols. By signing this agreement, the owner understands and accepts that such illnesses are an inherent risk of boarding and releases Maple’s Place from liability should an outbreak occur.

Transportation: If we transport your dog for veterinary care or pick-up/drop-off, Maple’s Place is not liable for accidents, injuries, or delays beyond our control.
Personal Property: Maple’s Place is not responsible for lost or damaged items such as collars, leashes, toys, beds, GPS collars, or other belongings.

Property Damage: Owners are responsible for any damage their dog causes to Maple’s Place property, home, furnishings, fencing, or equipment beyond normal wear and tear. The owner agrees to reimburse Maple’s Place for the cost of repair or replacement of damaged property.
​
Abandonment: If your dog is not picked up within 5 days of the scheduled departure after notice is given, the dog may be rehomed under Indiana law. You remain responsible for all fees and related expenses.

Photos & Media: Unless you opt out in writing, Maple’s Place may use photos or videos of your dog for business and promotional purposes.
​
Payment: All fees are due at or before pick-up. Boarding is based on a 24-hour period. 2–6 hours late = +50% of daily rate. Over 6 hours late = +100% of daily rateDeposits may be required for holidays or peak dates.

Booking & Cancellations: Cancellations made 3 or more days before check-in = no fee. Cancellations made less than 3 days before check-in or no-shows = one night’s fee. Holiday or peak bookings may require a deposit, which becomes non-refundable if cancelled within 7 days of the reservation.
Early pick-up is still charged in full unless otherwise noted at booking.
Assumption of Risk & Release: While every effort is made to ensure safety and comfort, dogs may become ill, injured, or die despite care. The owner assumes all risks associated with boarding, daycare, grooming, and transportation, and agrees to indemnify and hold harmless Maple’s Place, its owners, and employees from any and all liability, claims, damages, or expenses arising from their dog’s stay, except in cases of gross negligence or intentional misconduct.

Policy Updates, Renewals, & Future Bookings: agreement remains valid for all future visits and bookings made under the Maple’s Place Dog Hotel name. Future stays are subject to the most current policies and pricing in effect at the time of booking. Maple’s Place reserves the right to update or amend policies at any time, with reasonable notice provided to clients.

Arbitration: Any claim or controversy arising from this agreement will be settled by binding arbitration through the American Arbitration Association. Judgment may be entered in court. The arbitrator may award costs and attorney’s fees to the prevailing party.

VACCINATION REQUIREMENT:
By signing this waiver, I confirm that my dog is up to date on the following core vaccinations required for participation in boarding or daycare at Maple’s Place: Rabies, Distemper/Parvovirus (DHPP), and Bordetella (Kennel Cough). I understand that Maple’s Place may request documentation of these vaccinations at any time. I agree that failure to maintain current vaccinations, or to provide proof upon request, may result in services being declined. I also acknowledge that I am fully responsible for any illness or damage that occurs if my dog is not properly vaccinated and becomes sick or transmits illness to other dogs while in the care of Maple’s Place. Puppies under 1 year old are not required to have a Rabies vaccination.
